Class Registration Form

To register your child for a class, please complete the CLASS REGISTRATION FORM and turn it into the front office along with your registration fee.

Checking & Credit Card Draft Forms

To have your tuition automatically drafted each month please complete either the CREDIT CARD OR CHECKING DRAFT FORM. Drafts must be submitted by the first of the month. All drafts will be processed by the 5^th day of each month.

PS Camp Registration Form

To register your child for a preschool camp, please complete the PS CAMP REGISTRATION FORM and turn it in to the front office along with a $35 deposit for each camp.

Day Camp Registration Form

To register your child for a week or weeks of day camp, please complete the DAY CAMP REGISTRATION FORM and turn it in to the front office along with a $50 deposit for each week of camp.

Birthday Party Waiver

Rose's Gymnastics requires all birthday party guests to have a signed waiver form prior to participating in the birthday party activities. A waiver will be available at the gym on the day of the party for parents to sign as they arrive with their child. If you will not be attending or bringing your child to the party please fill out the BIRTHDAY PARTY WAIVER and send it with your child to the party.
